What’s New :
- Spouses will be excluded from our open enrollment for 2021. We hope to be able to offer private appointments for spouses in the near future.

IMPORTANT: In past year’s enrollments, we have screened almost 400 people in the last two days of enrollment. This is no longer possible due to space issues and safety procedures during the covid.
Enrollment Appointments
For our appointment scheduling to work during annual LYL enrollment, all appointment slots must be filled– including early in the month. We have a limited number each day.
- Be ON TIME to your appointment. If you are late, you may not be able to be seen.
- No walk-ins at LYL enrollment.
- You must have your fasting labs drawn the same day you have your biometrics measured.
- By being able to schedule your appointment, this will allow you to plan for your blood draw.
- Current participants: You will not be held to your 2020 Health Goal or Score from Interactive Health. You will receive 2021 premium discount as long as you complete enrollment by the deadline – whether you met your 2020 health goal or not.
- Look for your new, 2021 Goals/Metrics in November/December with our new wellness partner. This will be similar to what you’ve experienced before: blood pressure, lipids, glucose, and tobacco use.
